On a monthly basis you will be required to report all new checking accounts for
the match process. A reporting spreadsheet will be provided to you requesting the
following fields of information: Date Account Opened, Date of 1st Direct deposit
Transaction, Date of 2nd Direct deposit transaction, Name, Address, City, State,
Zip—in some cases we may require Account Type as a reported field.
